3. Provide information detailing your claim to ownership of the copyright in the allegedly infringing material:

Either:

a) Proof of copyright in the image or text concerned, i.e. proof of registration of the Image under the DMCA

OR b) without such registration:

1. Detailed description of the material claimed, AND
2. Evidence to support your claim that you own the copyright.

4. Include the following statement: “I swear, under penalty of perjury consistent with United States Code Title 17, Section 512, that the information in the notification is accurate and that I am the copyright owner or am authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.”

5. The above statement should be signed with an electronic or physical signature of the person authorized to act on behalf of the owner of the copyright or other intellectual property interest.
